Doing Therapy Differently: What Private Practice Has Taught Me

Working as a therapist in private practice offers a unique kind of freedom—one that allows us to shape our work in alignment with our values, strengths, and ethical standards. Over the years, I’ve come to appreciate this autonomy more deeply, especially in contrast to the constraints often found in larger organisations.

Autonomy That Honours the Therapist and the Client

In private practice, I choose my schedule, the clients I work with, and the therapeutic approaches I use. This isn’t just about convenience—it’s about integrity. Unlike in some organisational settings, where therapists are expected to work across all areas regardless of training or interest, private practice allows me to focus on what I do best. I no longer feel the pressure to be everything to everyone.

Let’s face it: we all have areas of therapy we’re more drawn to. When working for services like the NHS or IAPT, therapists often have to conform to rigid models—sometimes delivering therapy that feels more like ticking boxes than truly supporting clients. Six sessions to address complex issues? Surface-level interventions for deep-rooted pain? It can feel more about targets than transformation.

Confidence in Saying “No”

As my practice has matured, so has my confidence. I now understand the power of saying “no” when a client’s needs don’t align with my expertise. This isn’t rejection—it’s respect. It’s about ensuring they receive the best care, even if that means referring them elsewhere. In the early days, financial pressure made this harder. When your income depends on client volume, it’s tempting to take on everyone. But I’ve learned that ethical, effective therapy starts with fit.

The Many Hats of a One-Person Practice

Running a private practice means being more than just a therapist. I’m also the accountant, marketing manager, administrator, and lifelong learner. It’s a lot—but it’s also empowering. I stay up to date with training, manage my business, and maintain the standards I believe in. It’s not always easy, but it’s always worth it.

Looking Ahead

Private practice isn’t just a career path—it’s a commitment to doing therapy differently. It’s about honouring both the therapist and the client, creating space for real healing, and staying true to what therapy should be: ethical, client-centered, and transformative.
